Phitrum Surgery
Phitrum surgery is a cosmetic procedure designed to enhance the shape, length, or definition of the philtrum — the vertical groove between the upper lip and the nose. It can help achieve facial balance, create a more youthful appearance, and complement other facial features.

Philtrum Shortening Surgery

Step 1
Incision Design and Cutting: Make a curved incision at the junction of the columella base and alar root, hidden in the nasal base shadow.

Step 2
Skin Removal and Dissection: Remove excess philtrum skin and part of the subcutaneous tissue, preserving the orbicularis oris muscle to maintain natural lip movement.

Step 3
Lifting and Fixation: Lift the upper lip toward the nasal base and secure it to the nasal base or columella cartilage to maintain results and reduce incision tension.

Step 4
Suturing: Close in layers with absorbable sutures for deeper layers and fine sutures for the skin.
Philtrum Augmentation Surgery

Method A
Skin/Dermis Graft: Make small hidden incisions along the philtral ridges and implant autologous dermis, fascia, or synthetic material to create raised ridges.

Method B
Suture Suspension: Use subcutaneous sutures to tighten and gather the philtrum area, forming a defined central groove.

Method C
Soft Tissue Sculpting: Make a small incision, sculpt or remove subcutaneous fat to form a central shallow groove while preserving raised ridges on both sides; can be combined with lip contouring.
